This form is intended to provide BPL with proctoring information for individual students. Please follow these guidelines when completing and submitting the form.
1. The form should be completed by the instructor or testing coordinator, not the student.
2. A new form should be completed for each request for proctoring.
3. Any paperwork required by the testing institution, such as a proctoring agreement, must be submitted well in advance. BPL Librarians will complete and return the paperwork, provided it aligns with our policy and methods. The institution must approve any differences between its proctoring agreement and BPL policies before proceeding with the proctoring.
4. Proctoring agreements or other paperwork DO NOT replace this proctoring request form.
5. A fee of $25.00, payable by cash or check, is charged for each proctoring appointment and must be paid before the exam can begin.
6. It is the student's responsibility to follow up to ensure that proctoring appointments are made prior to the exam deadline, and to provide any necessary supplies or equipment.
7. While BPL endeavors to provide the best possible proctoring environment, limitations due to staffing and physical space exist. The instructor/testing coordinator must read the policies linked below, and initial that they are understood and agreed upon.
